Kaiser Case Managers

So, this post is for all of you who have loved ones with Kaiser Permanente. Just a reminder that I am not a lawyer or a medical professional, just someone who was trying to help her dad during a difficult time. I acquired some much-needed knowledge and wanted to pass it on to others who might be experiencing the same thing.
If your person is at a Kaiser hospital, don't pass up speaking with a Kaiser Case Manager, in addition to your follow-ups with your loved one's medical care team. For my situation, when my dad was at the hospital, the case manager's name and phone number were written on the whiteboard in my father's room. If this is different in your situation, ask your loved one's nurse or inquire at
the nurse's station for the contact number. Hopefully, they can help.
Depending on the care your loved one requires upon leaving the hospital, the case manager will help you navigate all of that:
Rehabilitation facilities
Equipment required upon leaving the hospital (If your loved one is going home, they can order durable medical equipment to be delivered)
Dialysis information
Hospice information
Next steps after leaving the hospital
In addition to so much more
Depending upon the Kaiser facility, they may also offer brochures for you to read when you have a quiet moment. Brochures are great since spoken information is a lot to take in all at once, even if you are taking notes. The case manager can also provide guidance and a shoulder to lean on when you are completely overwhelmed.
